How to Make a Referral
Thank you for referring your patient to Midwestern University’s Companion Animal Clinic. If you are referring a patient to one of our specialty services such as surgery, internal medicine, or neurology, please complete the online referral and submit all medical records, including imaging, to our clinic email at: AHI@midwestern.edu. Medical records can also be faxed to 623-806-7401.

Consultations
We are pleased to offer consultations to veterinarians regarding small animal internal medicine, surgery, and neurology cases. Please call 623-806-7387 and ask to speak to a small animal internal medicine, neurology, or surgical referral coordinator.
