How to Make a Referral

Thank you for referring your patient to Midwestern University’s Companion Animal Clinic.  If you are a veterinarian referring a patient to one of our specialty services such as Surgery or Internal Medicine, please complete the referral form and submit all medical records, including imaging, to our clinic email at: AHI@midwestern.edu.  Medical records can also be faxed to 623-806-7401. All questions can be directed to our referral coordinator by calling 623-806-7465.

If you are making a referral for Cardiology, please contact Valley Veterinary Cardiology directly at 480-828-0109 for an appointment at the Companion Animal Clinic.

DVM Consultations

We are pleased to offer consultations to veterinarians regarding small animal internal medicine and surgery cases.  Please call 623-806-7465 to speak to the small animal internal medicine and surgical referral coordinator.

If your patient is critical or requires emergency medical attention, please contact our front desk staff members at 623-806-7387. They will put you in contact with one of our clinical faculty members to ensure a smooth and seamless transfer.
