General Appointment Information
At the Companion Animal Clinic (CAC) in Glendale, AZ, we pride ourselves on offering high quality veterinary services with the prestigious responsibility of teaching Midwestern University’s 4th year veterinary students in the clinic setting. With that in mind:
- For general or specialist appointments, every patient will be overseen by veterinary students and one licensed veterinarian for comprehensive care. Please show our students patience and courtesy as the CAC is a positive learning environment and you are providing them with an opportunity to expand their medical education.
- At the first patient visit, you will be required to stay at the CAC so we can greet you and learn about your pet’s lifestyle and history. Please do not leave the clinic.
- Our appointments are longer than average to foster teaching, therefore plan to be at the clinic for 2 to 3 hours. We offer a large, comfortable lobby for waiting and provide coffee, water, and snacks. Feel free to bring any activities or work to do while you wait.
- Unfortunately, we cannot accommodate any requests for specific faculty veterinarians or students at any visit.
- In the future, convenient appointment options may include drop-off appointments, wellness-only visits, and technician appointments depending on your pet’s needs.
Appointment Confirmations
We ask that you confirm your appointment at least 24 hours prior through our text system or voice calls and understand that we may need to reschedule your appointment if we do not hear from you, or if you are tardy.
Medical Records
We will do our best to contact your previous veterinarian and transfer your pet’s medical record to our clinic. However, please bring records in your possession and any notes for your appointment.
Payment
Payment is due at the time of service. A deposit may be required with a treatment estimate or for any major procedures. If you have financial questions, our front office team can assist you.
Patient Pick-Up Reminders
All patients must be picked up from the clinic daily by 4:30 p.m.
Prescription Refills
Prescription refills should be requested for a minimum of 48 hours in advance.
Client Rights and Responsiblities
At Midwestern University Companion Animal Clinic, we are committed to providing patient-centered care that fosters the human-animal bond. We have the prestigious responsibility to educate our students to become Day One ready veterinarians.
At the Companion Animal Clinic you can expect:
- To be treated with professionalism, compassion, honesty, and respect — free from discrimination.
- Our priorities are your pet, your needs, education, and public safety.
- Knowledgeable and thorough veterinary care for your pet.
- To receive information that you can understand and to help you make the best choices for your pet.
- The ability to request a written prescription or your pet’s medical records.
- Privacy and confidentiality, unless you ask otherwise, or it’s required by law.
- To get notice from us, contact information for other veterinarians, and transfer of your pet’s care if we can’t treat your pet anymore.
What we ask from our patients is that our team of faculty veterinarians, clinic staff, and veterinary students are treated with professionalism, compassion, honesty, and respect — free from discrimination.
Be upfront and honest with our team, including:
- That you share the following information:
- Provide (or grant us permission to request) your pet’s complete medical records.
- Answer our team’s questions and give us any information you think is important about your pet’s health or our team’s safety (especially a history of biting, scratching, etc.)
- Sharing questions, challenges, or concerns about anything we’ve discussed, left unaddressed, or with any treatments.
- Only allow people whom you trust to get information, make medical decisions, and provide payment for your pet to accompany your pet to appointments. Due to time constraints, we generally cannot repeat conversations with multiple people.
Respect for our schedule and understand that medical care can be unpredictable.
- Understanding that after 3 no-show appointments or repeated tardies, during the preceding 12 calendar months, a patient may be dismissed from the teaching program or clinic. A no-show appointment is defined as the pet owner’s failure to provide a minimum of 24-hour notification of a cancellation and/or failure to show up for scheduled appointments.
- Understanding that being late to appointments and procedures by 15 minutes or more may result in us rescheduling your appointment to another day. Procedures may be cancelled and not rescheduled.

Provide payment at the time of service.
- A client may be dismissed for failure to meet all financial obligations.
