State of Illinois eye exam requirements

In Illinois, all children entering kindergarten or enrolling in an Illinois school for the first time are required to have a comprehensive eye examination performed by a licensed optometrist or ophthalmologist. The exam must be completed within one year before the start of school, and proof of the examination must be submitted to the school by October 15.
